Complete a bankruptcy form on behalf of someone else
A person can complete the bankruptcy form on behalf of the applicant only if the applicant is:
- blind, partially sighted, illiterate or partially literate
- non-English speaking or
- physically unable to complete the form
If one of the above situations apply, the person assisting the applicant cannot use AFSA's Online Services and will need to contact us to request a paper form.
For any other situation, including applications by mentally incapacitated persons (where the applicant is unable to complete the form themselves) please contact us.
This process doesn’t apply to registered insolvency practitioners or financial counsellors that are simply assisting someone with the bankruptcy process.
Steps to complete the bankruptcy form on behalf of someone else
When completing the form, you’ll need to complete Section M – Declaration of person completing this form on behalf of the applicant – non-online lodgements only.
Step 1: Contact us to request a paper form
Step 2: Make sure all sections of the bankruptcy form have been completed

Step 3: If you are completing the form on behalf of somebody else, you must complete the declaration in section M. You must:
- declare you have read the prescribed information to the applicant and
- select the reason they’re unable to complete the form themselves
Step 4: You (the person completing the form on behalf of the applicant) must sign and date the form and provide your name and contact details
We must receive the form within 28 days of the date you signed your form.
Step 5: Have the applicant’s identity verified by an authorised witness. You need to provide the identification documents of the applicant only, not your own identify documents
Step 6: Submit the form by email or post
