Registered Debt Agreement Administrators can use this form to provide information to debtors.

Form 36 - appointment of proxy. Complete this form if you wish to appoint a proxy to vote at a meeting of creditors and provide it to the trustee.

Form 27 - application to renew registration as a debt agreement administrator. A form for current debt agreement administrators to renew their registration.

Form 39 - application for determination of trustee remuneration. Trustees can use this form to apply to the Inspector General to determine remuneration.

This form can be used for enquiries relating to Annual administration return (AAR) support.

Form 32 - Annual trustee return. Trustees can lodge an annual trustee return through the online portal.

Form 11 - application for change of conditions on practising as a registered trustee (RT). A form for changing conditions as a practising registered trustee.

Form 25 - change of conditions on practising as a registered debt agreement administrator.
