This form can be used by those who are individual or company debt agreement administrators, and persons with overall management responsibility of a company.
Before applying, make sure you have all the required information to complete the application form. This includes:
- Details of any professional membership or disciplinary issues for the past 3 years (if applicable)
- National Police Certificate from the Australian Federal Police for an individual or a company and all directors of the company
- Evidence of certificate of currency
- Evidence of membership with the Australian Financial Complaints Authority
