Form 11 - application for change of conditions on practising as a registered trustee (RT). A form for changing conditions as a practising registered trustee.
Form 27 - application to renew registration as a debt agreement administrator. A form for current debt agreement administrators to renew their registration.
Form 23 - request refund of realisations (RC) or interest charge (IC). A form to request a refund for realisations or interest charges.
Form 39 - application for determination of trustee remuneration. Trustees can use this form to apply to the Inspector General to determine remuneration.
Use this form if you would like to request a review of your trustee’s objection to your discharge from bankruptcy.

Form 25 - change of conditions on practising as a registered debt agreement administrator.
Form 31 - request to renew registration as a trustee. If you are a currently registered trustee, use this form to make a request to renew your registration.
Form 18 - notice of special resolution. Trustees can use this form to notify AFSA of a special resolution passed relating to a composition or PIA.
Form 34 - trustee requests to the Inspector-General relating to registration
This form can be used by insolvency practitioners and creditors for support queries about AFSA’s debt agreements online platform.
Form 26 - application for surrender of registration as a registered debt agreement administrator (RDAA).
Form 35 - notice by an industry body of possible grounds for disciplinary action
You can submit a referral if you believe an offence may have been committed against the Bankruptcy Act 1966 or the PPSR Act 2009.
