Use this form to apply for certain information on the National Personal Insolvency Index to be withheld, removed or corrected if your safety is at risk.

About this form

If publishing certain information would/is likely to put your safety at risk, you can apply to have your address (and some other information) withheld from the National Personal Insolvency Index (NPII). We cannot withhold your name or date of birth.

You may also use this form to apply to correct or remove inaccurate or misleading information on the NPII.

Submitting the form

If you are applying for bankruptcy or have recently been made bankrupt by the court (sequestration order)

For online applications, make sure you upload this form at question 10 of your Statement of Affairs.

For offline (paper) applications, you must submit this form at the same time as your other required form/s. If you do not, your information may continue to be published on the NPII.

If you are applying for a debt agreement or personal insolvency agreement

To request to withhold your information on the NPII, your completed form must be submitted at the same time as your debt agreement / personal insolvency agreement paperwork.

If you are already bankrupt, or in a personal insolvency agreement or debt agreement

To request to remove or correct your information on the NPII, return your completed form to us using this online form: Send us a document.
