It's important you include all your debts when applying for bankruptcy. If you’ve forgotten to include any debts, you must notify your trustee as soon as you become aware.
Registered Debt Agreement Administrators can use this form to provide information to debtors in accordance with the Bankruptcy (Registered Debt Agreement Administrator Conditions) Determination 2020 before debtors sign a debt agreement proposal.
This form can be used in conjunction with Prescribed information - Debt Agreements.

This form can be used for enquiries relating to Annual administration return (AAR) support.
Trustees can use this form to lodge an annual trustee return. Trustees must lodge this return form annually within one month of the anniversary of their registration.
Form 39 Application for determination of trustee remuneration. Trustees can use this form to apply to the Inspector General to determine remuneration.
A form for current debt agreement administrators to renew their registration.
Complete this form (form 36) if you wish to appoint a proxy to vote at a meeting of creditors, and provide it to the trustee at (or preferable before) the meeting.
Assent / dissent form. Form to assent or dissent to composition proposal in bankruptcy. This form must be submitted to the trustee, together with a completed proof of debt form (POD), before the meeting.
