Application to join national panel of registered trustees form (ATJNPORT). A form for trustees to consent to act as trustee for bankruptcy estates transferred from the Official Receiver.

Pre-referral enquiry form. Practitioners use this form to submit a pre-referral enquiry (PRE).
If you are a currently registered trustee, use this form to make a request to renew your registration. Before applying, make sure you have all the required information to complete the application form, including evidence of a certificate of currency.
You will need to pay a renewal fee at least one month before the expiry date of your registration or a penalty of 20% will apply. For current fees, see Fees and Charges.
Form 33 - Trustee notification to the Inspector-General of significant and other events. Trustees can use this form to notify the Inspector-General of an event such as a change in registration details.

Trustees can use this form to lodge an annual trustee return. Trustees must lodge this return form annually within one month of the anniversary of their registration.

Complete this form (form 36) if you wish to appoint a proxy to vote at a meeting of creditors, and provide it to the trustee at (or preferable before) the meeting.
Form 38 - Notice of meeting of creditors. Trustees can use this form to notify us of a creditor meeting. This online form should be accompanied by the creditor meeting payment method form.
Common reasons to hold a creditors meeting include considering a composition offer, trustees informing creditors of conduct of the administration and to change trustees.
Form 39 Application for determination of trustee remuneration. Trustees can use this form to apply to the Inspector General to determine remuneration.
To enable the trustee to make an assessment of your ability to vote and the value of your vote you need to complete this form.
This statement of claim needs to be given to the trustee at (or preferably before) the meeting of creditors.
This form can be used for enquiries relating to Annual administration return (AAR) support.
This form can be used by insolvency practitioners and creditors for support queries about AFSA’s debt agreements online platform.
If you believe you are entitled to money from a previous or current bankruptcy, personal insolvency agreement or debt agreement, you can use this form to apply to claim that money back.
When you submit this form, you must have the relevant supporting documents ready to attach.
